MDA Health Plan Eligibility Verification Forms Now Due

Sponsors of the MDA Health Plan
should have returned their eligibility
verification materials to MDA Insurance
by Aug. 5. If you sponsor the plan in your
practice and have yet to return the
materials you received in July, please
complete the forms and submit them

Verifying eligibility is a standard part of offering a group health plan. It
provides a snapshot of your group at the point in time when the verification
is submitted. It is used to help MDA Insurance prepare each group’s renewal
documents for 2023. MDA Insurance is required to received your eligibility
verification documentation and auditors from the state Department of
Insurance and Financial Services spot-check for this documentation when
the health plan is audited.
Eligibility verification is a four-step process. First, obtain a Quarterly Wage
Detail Report ending June 30, 2022, known as form UIA 128, and complete it
as directed in the verification packet. Next, complete the highlighted areas of
the personalized Business & Participation Rules Worksheet that was included
in your packet. Third, obtain a waiver of insurance from every employee
indicated on the Quarterly Wage Detail Report with a W, which indicates they
are waiving coverage. New waivers must be obtained annually from anyone
eligible for the MDA Health Plan, but not enrolling in it. During open
enrollment in the fall, people who waived coverage for 2022 can apply for
coverage for 2023. Finally, return all the information requested in the
postage-paid envelope or fax it to 517-484-5460.
Delaying submitting the eligibility verification information will result in
delays in preparing your open enrollment information for the fall and will
give you less time to choose which plans to offer next year and complete the
open enrollment process. Please don’t delay completing the eligibility
verification process. And thank you to those groups that submitted the
required materials by the Aug. 5 deadline!
