MDA Health Plan Eligibility Verification Forms Due Aug. 6

MDA Health Plan sponsors should be certain to return their eligibility verification paperwork to MDA Insurance by Aug. 6. An eligibility verification is a standard part of offering a group health plan, and it is information that we are required to gather from each group. We must confirm all employees’ eligibility and enrollment. This documentation is the foundation for the preparation of 2022 renewal information, which will be sent to groups in October, and will include rates for next year. Here’s what to do:
1. Obtain your Quarterly Wage Detail Report ending June 30, 2021 (known as form UIA 1028): Payroll journals will not be accepted. Indicate on the UIA 1028 form who is part-time with a PT, who is full-time with a FT, and of those full-time employees, use a W to indicate who is waiving the coverage. If an employee is no longer with the office, please write Term next to the name.
2. Complete the highlighted areas of the Business and Participation Rules worksheet. Reponses from 2021 have been pre-filled for each sponsor, if applicable, to assist in form completion. To meet eligibility compliance, a sponsor mut have 75% of its eligible employees enrolled in health coverage somewhere. The number of full-time employees on the quarterly wage detail report, the number of waivers for qualified employees, and how many qualified employees are enrolled determine your participation percentage.
3. Obtain waiver forms from those on the Quarterly Wage Detail Report that have been marked with a W in step 1. Each year any employee who is eligible for the MDA Health Plan but who is not enrolling must submit a new waiver of coverage form. These employees will still have the option to enroll for Jan. 1, 2022.
4. Fax all completed forms to 517-484-5460 no later than Aug. 6.
MDA Health Plan open enrollment for 2022 begins Oct. 1 and concludes Nov. 15, 2021. Any changes to your plan may be made during open enrollment.
