It’s custom-designed for families, yet competitively priced for individuals and two-person families.
You won’t find a plan like this anywhere else. The plan is budget-friendly, with flat prescription costs and low copays. You’ll love the added  amenities such as health assessments, family-focused activity rewards, and even parental support to help reinforce positive habits and healthy living for kids. There are two Family Focus plans (HSA and PPO) available to best fit your needs.

2023 Plan Benefits

1. FAMILY FOCUS PPO

A Preferred Provider Organization (PPO) is a type of health plan that contracts with medical providers, such as hospitals and doctors, to create a network of participating providers. You pay less if you use providers that belong to the plan’s network.

    Employee: $3,000 ($4,000 co-insurance max)
    Employee +1: $4,500 ($8,000 co-insurance max)
    Family: $4,500 ($8,000 co-insurance max)
  • Out-of-pocket limit
    Employee: $8,700
    Employee +1: $17,400
    Family: $17,400
  • Coinsurance 80%/20%
  • Office visits $20 copay (deductible does not apply), including chiropractic visits.
  • Specialty office visits $40 copay (no deductible), including physical therapy
  • Virtual office visits $20
  • ER $100 copay after deductible
  • Urgent Care $50
  • Chiropractic $20 (limit 24 visits per year)
  • Pharmacy $10 generic / $20 non-preferred generic / $60 preferred brand  / $120 non-preferred brand.
  • Specialty Pharmacy 20% up to $200 for preferred specialty / 50% up to $500 for non-preferred specialty.

2. FAMILY FOCUS HSA 

A health savings account (HSA) is a tax-advantaged medical savings account available to those enrolled in a qualified high-deductible health plan (HDHP). The funds are contributed to an HSA on a pre-tax basis and can be spent on qualified services tax-free. Percentages show what you pay after your deductible is met:

    Employee +1: $6,000
    Family: $6,000
  • Out-of-pocket limit
    Employee: $7,050
    Employee +1: $14,100
    Family: $14,100
  • Coinsurance 80%/20%
  • Office, Specialty, Virtual, ER, Urgent Care visits:  You pay 20% after deductible is met.
  • Chiropractic 20% after deductible (24/year)
  • Pharmacy Maintenance medication for certain chronic conditions are covered prior to the deductible. Copayments will apply. All other prescriptions are subject to deductible.
    $10 generic / $20 non-preferred generic /$60 preferred brand  / $120 non-preferred brand.
  • Specialty Pharmacy 20% up to $200 for preferred specialty / 50% up to $500 for non-preferred specialty.

Both plans feature these exclusive benefits

  • Positive habit reinforcement for kids. Ask about our incentive plans that reward kids for making healthy life choices by limiting screen time, exercising daily, and making healthy eating choices.
  • Activity Rewards. An opportunity to earn up to $120 per year for just being active.
    For those 18 and over, earn 180,000 points per month for a $10 gift card.
    For those 17 and under, earn a $10 gift card with minimum of 400 points per month for exercising, writing book reports and making healthy choices.
  • Free Health Assessment. Assess your overall level of health by completing a confidential online health assessment for yourself and your spouse (if enrolled).
