1. LIVING FIT HSA

A health savings account (HSA) is a tax-advantaged medical savings account available to those enrolled in a qualified high-deductible health plan (HDHP). The funds are contributed to an HSA on a pre-tax basis and can be spent on qualified services tax-free.

  • Deductible $3,000 / $6,000 / $6,000
  • Out of pocket limit $7050 / $14,100 / $14,100  (but not to exceed $6,900 per person)
  • Coinsurance 80%/20%
  • Office visits 80% after deductible
  • Specialty office visits 80% after deductible
  • Virtual office visits 80% after deductible
  • Emergency room visits 80% after deductible
  • Urgent Care 80% after deductible
  • Chiropractic 80% after deductible   (limit 24 visits per year)
  • Pharmacy Maintenance medication for certain chronic conditions are covered prior to the deductible. Copayments will apply. All other prescriptions are subject to deductible first. $10 generic / $20 non-preferred generic / $60 preferred brand  / $120 non-preferred brand copays
  • Specialty Pharmacy 20% up to $200 for preferred specialty / 50% up to $500 for non-preferred specialty
  • Exercise Rewards An opportunity to earn up to $120 per year for just being active. Receive discounts at fitness clubs and exercise centers within the ExerciseRewards network.

A Preferred Provider Organization (PPO) is a type of health plan that contracts with medical providers, such as hospitals and doctors, to create a network of participating providers. You pay less if you use providers that belong to the plan’s network.

  • Out of pocket limit $8,700 / $13,875 / $17,400
  • Coinsurance 80%/20%
  • Office visits $30
  • Specialty office visits $45
  • Virtual office visits $30
  • Emergency room visits  $100 copay after deductible
  • Urgent Care $50
  • Chiropractic $30 (limit 24 visits per year)
  • Pharmacy $10 generic / $20 non-preferred generic / 50% preferred brand (max $100) / 50% non-preferred brand copay ($300 max)
  • Specialty Pharmacy 20% up to $200 for preferred specialty / 50% up to $500 for non-preferred specialty
  • Acupuncture Services $30 copay up to 20 visits. Deductible does not apply.*
