Enrollment Form Explanation

In late January, MDA Insurance mailed information about the new voluntary Allstate Benefits accident and critical illness insurance programs. The critical illness policy is temporarily offered on a guaranteed-issue basis—meaning everyone who applies will be accepted— which is a great opportunity for members who have pre-existing conditions to get coverage they may otherwise be deemed ineligible to receive. Because of the guaranteed issue provision, and to ensure that every member received the opportunity, Allstate Benefits asked us to have all members to complete the enrollment form to opt in or decline coverage.

We apologize for any confusion caused by this request and assure you that no one will be required to pay for insurance coverage they do not elect. If you choose not to return the enrollment form indicating your election by March 16, we will assume that you decline coverage. Thank you for your cooperation and understanding as we strive to bring you programs that protect your financial well being.
