You & the New Health Care Claims Tax

Effective Jan. 1, a 1 percent tax will be applicable to all health insurance claims made in Michigan by Michigan residents. Dental and vision claims are subject to the tax, as are nearly all medical-related claims. Blue Cross Blue Shield of Michigan and BCS Life will begin applying a 0.75 percent factor to plans as a line item on each premium statements. VSP, which provides vision insurance to MDA members, is formulating its response to the Michigan claims tax. A gentle reminder

MDA Insurance quarterly billed medical coverage premiums are due on the first day of the month of each quarter. Don't let your coverage lapse-it's just too important! If you have any questions, regarding your payment, please contact us directly at (800) 860-2272.

Important Note

If employees leave your office and you need to cancel their insurance, please don't forget to cancel their Delta Dental coverage (if they have this insurance) in addition to their BCS individual coverage. Both contracts need to be cancelled in writing. A fax or letter sent to MDA Insurance will ensure it's done in a timely and efficient manner. Is Your Newborn Covered?

If the stork visits your house and you're a Blue Cross Blue Shield subscriber, be sure to notify MDA Insurance when your baby is born. This involves filling out a membership change form and paying an additional premium to cover your little one. Just contact MDA Insurance within the first 10 days after your baby is born. We'll mail you both the necessary membership change form and the balance of the quarterly premium due. Both items must be submitted together so the records can be changed to reflect your new addition. If you have questions, please give us a call...and congratulations on your new addition!
